QIDP/Social Work/Case Manager

What is a QIDP? A QIDP is vital to ensuring that the clients served by ADEC are receiving the quality services needed to live a life full of informed choice and possibility.
A QIDP advocates for the clients' needs and wishes, facilitates development of the Person Centered Individualized Support Plans (PCISPs), assesses if the PCISP is implemented properly, and coordinates discussions with the client and their care team to maximize success.
This position would be a good opportunity for someone interested in social work, psychology, or experience with the mental health field or the school system.

Job
Responsibilities:
Facilitate and oversee all management and creation of the Person-Centered Plan process for assigned caseload.
This includes creating, monitoring, and reviewing assessments.
Ensure quality of care to individuals served through direct contact with individuals served and their staff.
Maintain all client based trainings in solana programs Assist will compliance with Indiana Department of Health, Life Safety surveys and other Group Home regulations Assist with new referrals and providing feedback to find the best fit for new referral Job Requirements:
Bachelor's degree in human services fields required At least two years' experience in direct programming for individuals with disabilities Ability to lift 60 pounds Valid driver's license Proof of car insurance Pass background and negative drug test Why Should you Apply?ADEC has been providing services to children, adults and families with developmental disabilities for nearly 70 years.

$1000 Retention Bonus:
$200 at 90 days, $300 at 6 months, and $500 at one year.
